Referrals
A prescription for evaluation and treatment must be received
from the referring physician prior to the scheduling
of an appointment. Referrals may come from primary care, or
specialty physicians, parents or early intervention
providers. Referrals should be made through a centralized
referral line, 860.545.9889.
Prior to the appointment, intake information is gathered to
determine which professionals should be directly involved in
the child's assessment (Occupational Therapist, Speech
Therapist, Nutritionist, Psychologist and/or Developmental
Pediatrician). Coordinated scheduling of appointments with
the designated disciplines generally enables the family to
make one visit.
