Pre-Operative Checklist

Your doctor has determined that your child needs surgery. The surgery will be performed at the Connecticut Children’s Medical Center in Hartford, CT. There are a few things you will need to do to prepare for the day of surgery. Please use this checklist to ensure that things go smoothly on the day of surgery.

Hospital Info You have been given a large white envelope with information from the Day Surgery Unit. Included in this is a form that must be filled out and returned to the Pre-op office in the postage paid envelope provided. There is also information about hospital tours. The tour is a wonderful way to prepare both you and your child for your planned surgery.

Insurance Please be sure that our office has your correct insurance information. We will handle all necessary pre-certification. If you have any insurance changes prior to your scheduled surgery, please notify us as soon as possible. You may wish to check with your insurer to see if a second opinion is required prior to surgery.

Physical Please make an appointment with your child’s pediatrician for a preoperative physical. This must be done within the 7 days prior to your surgery date. The “Short Stay” form included in this package must be filled out by your child’s doctor and faxed to the Pre-op Office at (860) 545-9888. You must also bring the completed form with you on the day of surgery. If the physical is not done before your child’s procedure, their surgery may be cancelled. If your child is ill within the week before surgery, please notify both the Pre-op office at (860) 545-9890 and our office at (860) 545-9520.

OR Time You may call the Pre-op Office two business days before surgery for your scheduled time and feeding instructions. Please make sure that our office has your correct daytime phone number, so that the Pre-op staff can reach you. If your address or phone number changes prior to your surgery date, please notify us as soon as possible.
